The link comes from the Invite email.  This will bring the Interviewer(s) to the Feedback Form that was selected.

Interview Kit example

The top of the form provides a Welcome message and high level details about the Interview/Meeting.

Completing the Feedback

The interviewer(s) will complete the form.   There are also widgets to help keep track.
