This article covers the setup for adding, removing, or editing your information for: Vaccines, Exemptions, or to select the Testing Option.  Some options might not be available.  Please contact your Staffing Agency with any additional questions or assistance.

Note: Limited users at the Staffing Agency are able to view your uploaded documents.

My Ready Screen

Click on the My Ready tab

My Vaccination Status

Select your Vaccination status, Exemption, or choose the Testing option.

Note: Some options might not be available based on your Staffing Agency's setup.

1. Click the Update My Vaccination Status button.
   
   
   
   
2. Click on your response.
   1. I am Fully Vaccinated
   2. I am Partially Vaccinated
   3. I am Medically Exempt
      Note: This option may not be available and is set up by the Staffing Agency.
   4. I am Religiously Exempt
      Note: This option may not be available and is set up by the Staffing Agency.
   5. I am choosing the testing option
      Note: This option may not be available and is set up by the Staffing Agency.
      
      

I am Fully Vaccinated

• Click the OK button

I am Partially Vaccinated

• Click the OK button

Adding data

1. Enter data into the following fields:
   1. Vaccination Type (Required):
      1. Pfizer-BioNTech
      2. Moderna
      3. Johnson & Johnson
   2. Dose 1 Date (Required):
   3. Dose 2 Date:
   4. Booster Date:
      
   5. Validated:
   6. Details:
   7. Document Type:
      1. Vaccination Document
      2. Identification Document
         Note: The Document Type may not be listed.
   8. Click the Submit Vaccine Details button

Uploading a document

1. Locate the document on your device.
2. Click the document
3. Click the Open button
   1. Limitations:
      1. File size limits; 5 MB
      2. Formats
         

         PDF

         Docx

         PNG

         JPEG

         TXT

   3. If a Document isn't attached then a warning message will appear:
      1. Cancel = allows you to upload a Document
      2. OK = submits the Test without a Document
         
         

Completed Vaccine example

1. Click the Show button to display more information
   
   
   
   
2. Click the hide button to not display the details
   
   

Add new Vaccination

We recommend a new Vaccination record for each Vaccination card.  This may help manage, and validate, individual records instead of editing existing Vaccination records.

1. Click the Add a New Vaccination button
   
   
   
   
2. Enter the data again
   Note: If you are entering information for Dose 2, or Booster we recommend entering the Dose 1 Date in the Dose 1 Date field.
