Easily obtain interview feedback from managers using Feedback Forms. These forms can be included in a submittal email, or when you send an interview kit.
The feedback form will be included with the email in link format. Once the manager submits their feedback, you will be notified via email, and the form will be stored on the applicant profile under the feedback form tab.

Creating a Feedback Form
Create up to 100 questions using up to 5 multiple choices options or free text/essay fields. Although you are able to edit the questions, we recommend not making changes once the Form is in use.
- Click on Account & Settings
- Click on Utilities
- In the Search field, type: FEEDBACK
- Click on the Manager Feedback Forms tile
- Click on the Create a New Feedback Form button
- Enter a name for your Feedback Form, in the New Form Name field.
- Click on the Create button
- Click on the Introduction Text field.
- Type an introduction/welcome message for the User.
- Click on the Update Name and Intro button
- Click on the Add Question button
Adding Questions
There are 2 options for adding questions (Multiple Choice and Essay).
Note: We recommend not making changes once the Form is in use.
Multiple Choice
Ensure that the Question Type field selected is Multiple. You are able to enter up to 5 options for multiple choice. Any empty field will not be shown in the list.
- Fill out the fields listed below
- Question: Enter the question the Interviewer should answer
- Question Type: Select the Multiple option
- Question Order: Select the order in which the question is asked.
- Answer 1: Enter an option for the User to choose from
- Answer 2: Enter an option for the User to choose from
- Answer 3: Enter an option for the User to choose from (optional)
- Answer 4: Enter an option for the User to choose from (optional)
- Answer 5: Enter an option for the User to choose from (optional)
- Click on the Save Edits button.
Essay/Free text
Ensure that the Question Type field selected is Essay.
- Fill out the fields below
- Question: Enter the question the Interviewer should answer
- Question Type: Select the Essay option
- Question Order: Select the order in which the question is asked.
- Click on the Save Edits button
Additional options
Rearrange questions
We recommend adding all the questions before changing the order.
- Click on the Edit icon
- Click on the Question Order field
- Change the value to the new position/order.
- Changing the position/order doesn't change the original question it's replacing.
- Click on the Save Edits button
Saving Feedback Form
Once you've added, and rearranged the questions if necessary, then be sure to save your changes.
- Click on the Done Editing button
- Now you'll see your Feedback Form in the list of Forms
Editing a Feedback Form
The following message will appear at the top of the Feedback Form when editing:
- Click on Account & Settings
- Click on Utilities
- In the Search field, type: FEEDBACK
- In the Search field, type: FEEDBACK
- Click on the Manager Feedback Forms tile
Delete a Question
- Click on the Edit icon, for the Feedback Form you'd like to edit.
- You can also search for your form in the Search for Feedback Forms field.
- You can also search for your form in the Search for Feedback Forms field.
- Click on the Delete icon, for the question you'd like to remove.
- A window prompt will appear and ask you to confirm you want to delete.
- Click on the Delete button

- A windows prompt will appear and ask you to confirm that you want to delete the Form.
